This guide will help GPs and their primary health care team colleagues to:
- Stimulate earlier identification of cases to help the patient and carer come to terms with diagnosis and prognosis
- Provide an easy method of assessment, enabling the primary health care team to make collaborative plans for future services
- Reach the correct diagnosis and detect any remedial pathology
- Carry out a review of the patient's medication
- Assess the patient's functional state and encourage optimum functioning
- Anticipate and avert crises by assessment of need of social and domestic circumstances
- Trigger the provision of social support and make appropriate referrals to other agencies
- Examine the needs of carers and find ways of providing them with information and advice for the future
